Immunizations and Vaccines
Michigan parents or guardians opting out of the vaccinations required for children to attend childcare or school, must request a waiver from their local public health department. Waivers are required for children enrolled in kindergarten, 7th grade, new to a school district, or childcare whose parents opt not to vaccinate.
If an unvaccinated child is exposed to a vaccine- preventable infection, or case(s) occur in the school or childcare, the child may be excluded from attending for the duration of the communicable period.
The health department CANNOT issue Medical Waivers. A medical waiver is issued by a physician and submitted to the school or childcare when a child cannot receive vaccine for medical reasons. Waivers issued by the health department are for parents choosing not to vaccinate based on religious or philosophical beliefs or other concerns.

Health Care Plan Forms
If a student requires a health related plan while in school the appropriate form must be completed and submitted to the school.
Medication Forms
Students requiring medication during school must have an authorization form on file with the District.
Self-administering Medication
Students self-administering medication must have the Medication Authorization Form-Physician/Parent Signature for Self-Administration/Self Possession form on file.
Staff Assistance with Medication
Students requiring staff assistance with medication must have the Medication Authorization Form-Physician/Parent Signature form on file.
Head Lice Information
In the event your child has head lice, please report it immediately to the school office. In order for the student to return to school, the District Health Aide must confirm that there are no live head lice. Parents can bring their child for a head lice check to the Board of Education office at 639 Oak Street between 7:30 and 8:30am (school days only). Families can call ahead at 734-759-6025 to confirm Health Aide availability.
If the Health Aide confirms the presence of live head, the student will need to be checked again in 10 days to ensure there are no live lice. Students with head lice cannot return to school until the District Health Aide confirms there are no live head lice.
