Special Dietary Request Procedure

If a special dietary request is necessary, families can submit a Request for Special Dietary Needs Accommodations:

1. First, visit Michigan Department of Education Special Dietary Needs and download the Request for Special Dietary Needs Accommodations PDF file.
2. Complete the form.
3. Next, a licensed physician, physician assistant, or advanced practice registered nurse must complete the Required Signature section.
4. Once the form is filled out and signed by a doctor, return it to the school's main office.
