Employer-Provided Health Insurance 1095-C

The Federal 2024 ACA Reporting and Enforcement Rules have changed so that Applicable Large Employers are no longer required to furnish 1095-C forms to all full time employees, except if requested by the employee.

Request a Copy of the 1095-C

An employee may request a 1095-C form by calling 734-759-6006 or mailing Wyandotte Public Schools, c/o Benefits, 639 Oak Street, Wyandotte, MI 48192. Once the request is received, the District will provide a copy of the form within 30 days or by January 31.
